Can someone tell me how you are billing Medicare for 88305 and 88307 please for Iowa or point me in the direction of their 'rules' for this since there isn't an official published MUE for either of these.
Here's some examples of how my billers said they have to be entered:
1. 88305 x 3 with 88305 x 2-59
2. 88305 x 3 with 88305-59
3. (other cpt but example that I don't understand) 88311 x 3 with 88311 x 2-59
4. (another cpt) 88175-59 (Seriously this was billed out alone, just like this)
I'm having a really hard time getting this. Wouldn't the correct way be:
1. 88305 with 88305 x 4-59
2. 88305 with 88305 x 3-59
3. 88311 with 88311 x 4-59
4. 88175 and NO 59 modifier
Am I missing something about billing units that's funky?
