In a radiation oncology follow up if a patient no longer has any evidence of disease say breast cancer (174.0-174.9) you use the history of code V10.3, but say the patient had carcinoma in situ of breast (233.0) but now has no evidence of disease what code do you use?
 
If the cance is still neing treated then it is active, for breast ca if the patient is being treated long term with tamoxifen then it is still active ca 174.9/ In situ ca is hx of ca after the excision is completed.
