Wiki Dx code when used as part of decision making

PLEASE HELP!
I have an ear nose and throat doc.
Example:
CC: Epitaxis

Doc gives me dx of epitaxis AND Diabetes xxx because he used the diabetes as part of his decision making.
My question: Do I bill out with the DX code for Diabetes even though he wasn't TREATING the diabetes that day? Or, should I instruct him to notate it in his records but it should not be part of billing?

Any help is so greatly appreciated!!!!
