What would be the best dx code for DIALYSIS MAPPING? CPT used is G0365

Diagnosis for Dialysis Mapping

If they are doing dialysis mapping, I am assuming that the patient has kidney disease and you would have to code according to the level. If it is end-stage, 585.6...if it is stage 4 (for example), 585.4.
