Can anyone clarify the correct coding for metastatic disease. We will use breast cancer as the primary diagnosis 174.9. If the patient has metastatic disease to another organ the cells that have metastasized are actually breast cancer cells. So as an example lets say the patient has metastatic disease to the lung. The cells that have metastasized are breast cancer cells and not lung cells so do you use the secondary code 198.81 for breast or do you use the secondary code for 197.0 for lung.... How do you identify where the cancer metastasized to? Have just had a total brain block and can't seem to get this...any help is appreciated.
 
you would use the secondary lung ca code to show that is the secondary site for the primary malignancy and then use the breast code as the 174.9 or the V10.3 whichever meets to show the primary site.
