DX coding for mastectomy after cancer

Patient has had a mastectomy due to cancer (ER +) and the treatment is now for the chest wall and axillary lymph nodes. What is the proper DX code.
Is it 173.50, V86.0 196.3 or is it 195.1, 195.3, V10.3.
The diagnosis does not istate mets to the chest wall.
 
The question you pose is a bit vague. When you say treatment is now for chest wall and lymph nodes, what kind of treatment. If they are treating cancer in these locations due to the breat cancer then they are metastatic sites. The V45.71 code cannot be used first listed. If treatment is still directed at the breast site then the cancer is still coded active, if the breast site is no longer being treated then it is history of. So you need to say what the current treatment is and if it stated that these sites are due to the primary.
