The ASC I work for has an ophthalmologist that instead of writing/typing out the diagnosis he is just placing the ICD-10 (ex. H25.12) on the reports. I was recently at an E/M Seminar that stated this is a big NO. Can anyone forward me to documentation to support that he is unable to do this and is therefore not compliant?? All I'm finding pertained to E/M coding and not specifically ASC.
 
AHA Coding clinic 1st quarter 2012 states the provider is not allowed to state the diagnosis using the ICD code. The provider is required to render the diagnosis "in his own word". You can purchase this coding clinic if you need to.
