need some information about documentation.

Established patient
chronic conditions
chief complaint ---- sore throat

Primary DX 462
followed by chronic conditions not necessarily address by doctor but notated because patient still suffers with them.


This is OK without further workup on chronic conditions correct.

Sorry if it's confusing
 
You usually only want to code chronic conditions when they apply to the patient's treatment or affect the patient's treatment in that visit.
