How should we order the diagnosis for a child patient who comes in for routine preventive visits when they have a stable chronic condition (asthma) that doesn't require any additional work-up, in reference to the primary, immunization, and counseling codes?

For example:

Child patient comes in for preventive visit unremarkable. Mild asthma stable. Dietary counseling. Immunization given.

I feel in this case the code order should be:

Z00.129 - Encounter for routine child health examination without abnormal findings

Z23 – immunization admin

Z71.3 – dietary counseling

J45.20 - Mild intermittent asthma, uncomplicated

The immunization and counseling codes are more related to the service provided, in my opinion.

The order listed is correct. Z00.129 is PDX for preventive visit. However, the abnormality was listed on A&P. You may consider change PDX Z00.129 to Z00.121
