In the assessment and plan the provider stated that patient is a male with unspecified depressive disorder, transferred from another hospital. Is the diagnosis major depressive disorder recurrent F33.9? or f32.9?
 
F32.9

I would use F32.9. The notes for F32.9 state, "Depressive disorder NOS" because the physician didn't document "recurrent".

However, whenever in doubt your best bet is to query the provider. Never guess or make an assumption regarding the physicians intent.
