I have a pt seen in office before chemo tx. Dr has dx listed in chart note as:

CA Mets to Intrathoracic LN 196.1 (primary dx)
Lung CA, L Upper Lobe 162.3 (secondary dx)

The patient is having chemo for the Lung CA. Should the primary dx be 162.3Lung CA, L Upper Lobe?

Is there a reference for coding dx sequencing?
