When the provider documents pregnant patient is having spotting and cramping. Is cramping a symptoms of spotting? Do I just use O26.85X? or do I code cramping R10.2?

Cramping is not a symptom of spotting as you can have spotting without it. Feel free to indicate both codes. But I would think you would want to report 026.89_ for the cramping instead of just R10.2 (or report R10.2) as a third diagnosis.
