dysplasia of the anus

I am having a hard time getting claims paid for a patient that has a diagnoses of dysplasia of the anus - K62.82. Geisinger insurance isn't paying for E/M 992.. or for an exam sigmoidoscopy 45300 or anoscopy 46600. Any suggestions?
