Wiki "E" codes on first claim submission only ??

Need your help out there and this may be a needle in a haystack. I think that I remember reading somewhere that the accident "E" codes should be placed on the claim with the intial treatment only. If this is true, does anyone know where to find it? Thanks for your input!!
