Wiki E/M, 58301, G0101 Can we bill it together? What modifier do we use

Hello,
I need help. Our provider is billing E/M with a modifier 25, 58301-59, and G0101 together. E/M and 58301-59 is denied but the G0101 is bundling with 58301. Are we suppose to remove the mod 59?
