I have a patient that was seen for acute vaginitis, testing for HIV and testing for STD, my provider wants to bill a 99203, 86703 and 87110, does the E/M get the 25 modifier?
 
Hi there,
You should only use a modifier when the services are bundled and there's legitimate reason to use a modifier to unbundle the services. You'll need to check the payer's bundling edits to see if those codes are bundled. If you're billing Medicare or a payer that uses NCCI edits you can use a MAC's NCCI tool.
