Wiki E/M AND impairment rating??

I have an office manager and physician for one of the offices I code for, that want me to bill for an office visit (99203), as well as an impairment rating (99456). I have never encoutered this before, but it just does not seem ethical to me. Can someone correct me, educate me, etc?? :)
