We are using Medicare's E/M Ducomentation Auditor Sheets. When choosing the correct established office code it states that it requires 2 components within the same column. How do you code if you do not 2 components in the same colomn?

E/M for established patietn

2:3 is in reference to the need to meet or exceed 2 out of 3 of the key components: History, Exam and MDM. For a new patient 3:3 must be met or exceeded.

If the components are not in the same column then they default to the lower one since it must meet or exceed the component to be counted.

For example: established pt--- Exam is EPF and MDM is M, I would go with 99213 since the Exam does not meet the level for 99214 even though the MDM does.
