HI,

If patient has come with GI bleed with Dr.XYZ for first time then we will code from 99201-99205 and same patient will come next month with Heart failure to same physician then what E/M level we can give?
 
Condition is irrelevant. if you have a CPT book it will give you the new vs established criteria. There is a nice flow chart. Its who they see, their specialty and how long its been since they were last seen
