Are there any audit sheets for nursing home charges? I am having a hard time finding anything that will work.

This may not be exactly what you're looking for as isn't really an 'audit sheet', but I work off of our Medicare contractor's (NGS) E&M tool which you can find at the link below. The nursing home code levels and required documentation levels are on page 6:
