I am looking for documentation specifically for E/M coding in an Inpatient Psychiatric setting using 1995 guidelines. Any suggestions?
 
What type of documentation are you looking for? I've found that for psychiatric coding the providers usually do better under 97 specialty specific guidelines. Under 95 most of the time I only get 2 or 3 organ systems (constitutional, psych and sometime M/S) while under 97 the specialty specific exam is geared directly to psych and they get a higher level exam.
