Tricky questions

I would add to your list: If a diagnosis is known, and is listed under assessment, when is it considered being treated/addressed? If it is under "Assessment" section, doesn't that mean we can bill it? Is there a difference between assessed and addressed? If hypertension is listed, has it been addressed by blood pressure taken by Medical Assistant and can it be billed? If hyperlipidemia is listed because it was found in high cholesterol values in labwork, can this be billed?
Is that what is meant by managed in Evaluation and Management to determine levels? What exactly is meant by management?
I know if Rx is given, that diagnosis can be billed because it is being addressed. What are some other examples of chronic or longstanding or intermittent problems that can be billed if no Rx is given?
