Wiki E/M coding with a Physician and a PA

How would I code an E/M when the H&P, CC, PFSH, ROS is done by the PA and then the physician does her own exam and documents this separately? Do I add both of them together to determine the code?

Are they trying to bill this as a split-shared visit, and is the payer Medicare?
