A newly joined GP physician in our facility having old patients from his previous facilities.

But they are new for our facility old for him(less Than 3 Years), so which category this patients will belongs....New or Established ?

Hi

When the patient new to your facility (new visit or patient had not visited the facility for more than three years), it should be billed as new patient - even when the patient is seen by your new GP at his old facility where he was serving before

It doesn't matter where he sees the patient. Established patient status follows him to the new practice.

If the patient sees someone else in the group practice for their first visit to the practice then it's a new patient.

Same group practice refers to when a patient is already established to the practice and say they see a different physician the next time they are only considered new if the physician is a different specialty and sub specialty. Like when another provider fills in or a new provider joins the practice they are still considered existing due to the specialty rule
