HI, can anybody tell me whether we consider "No loss of consciousness as associated sign and symptoms or severity. I have strongly doubt in this scenario . As patient having SW of abdomen , chest and NO LOC document.

Actually my auditor telling we not consider no LOC as severity it it should be come under sign and symptom. By I referred one book conquer of medical coding in this one case study says we consider No LOC as severity.

So just clarify whether I need to take LOC as servity or sign and sym and why?
 
Sign/Symptom but I would ask the provider if the intent is to convey severity. He/She may be documenting that for severity since significant internal bleeding can lead to loss of consciousness
