My question to you all, how would a physician document a follow up on a abnormal mammogram??? To justify a level of service for a E/M.

Could you be more clear? Are we coding for the Radiologist who performed the Mammogram or for a Physician who belongs to a different speciality?
 
It's a visit done by the Family Physician. Follow Up, Going over the abnormal mammogram.

How would I direct the physician on what's needed to code an E/M.
