Can anyone please tell me if there is anything in the guidelines that states specifically how many vital signs are needed to charge a new or established visit? If so where is it stated?

A minimum of three (3).

I have always seen 3. Take a look at the 1997 guidelines, when choosing the vital signs, it states "any 3 of the following seven".....
