I know that Medicare will not accept "noncontributory" as documenation in the HPI but what about private insurances? My gut is to follow the same guidelines as Medicare but I want to be certain.

Oh and what if you have a pt that can't speak due to severe brain injury recently sustained so physician documents "unotainable" for ROS. Does he get credit for that?

All and any input is greatly appreciated.

Medicare guidelines are usually the strictest so following them is safe. This may also be a question for the insurance carrier specifically.

Yes you would get credit as long as it is documented in the chart
 
It depends on the carrier.

WPSMedicare does not accept "noncontributory" and they do not give credit for unobtainable elements. They only give credit for what you do.
