When is it appropriate to code a level 4, 5 or 6 for a hospital patient, both Observation and Inpatient? Is it only upon discharge, whether same day from observation or discharge from inpatient? Can these levels ever be coded for subsequent hospital visits while still inpatient?

Can you please rephrase your question. It is difficult to know what you are asking. When you say level 4, 5, or 6 do you mean 99224-99226 subsequent observation codes?

As a general answer to your question, you bill appropriate E/M codes when supported by the documentation.
