When a doc sees a pt for a mole removal, and discusses a couple different things and prescribes for them, would you attach the mole dx to the E/M as well as the procedure code or just the procedure code? I've gotten different answers from people.
 
If you are billing a procedure and an E&M code with a modifier than the mole dx would only be associated with the procedure and not the E&M as you are stating that they are two separately definable services.
