if a patient saw a physician assistant for the first time and was a new patient, and she refers them to one of our orthopedic doctors can he bill for a new patient or is is an established?

If the PA and the physician work in the same office then no. The patient is only considered a "new" patient on the first visit. All subsequent visits would be considered established.

The patient would be new to Ortho if the PA is not of the same specialty.
