A patient is brought by EMS to the hospital with multiple stab wounds to the chest. He's seen by the Trauma Surgeon in the ED and after evaluating the pt he decides to admit him. Should he bill using the E/M Emergency Department Services codes 99281-99285 or the E/M Initial Hospital Care codes 99221-99223? Please provide references for your decision.

Initial Hospital vs ED Services

I would choose the Initial hospital care codes.

CPT (Under the heading of INITIAL HOSPITAL CARE) states:
...."When the patient is admitted to the hospital as an inpatient in the course of an encounter in another site of service (eg, hospital emergency department, etc...) all evaluation and management services provided by that physician in conjunction with that admission are considered part of the initial hospital care when performed on the same date as the admission. The inpatient care level of service reported by the admitting physician should include the services related to the admission he/she provided in the other sites of service as well as in the inpatient setting...."

This is just my opinion. :)
