optometrist wants to bill exam based on time 1 hour discussion. can he bill 99215 or does is have to be only eye codes 92012,92014?

I agree with Cheezum51, I work for a big organization and we also bill E/M for Optometrists based on MDM or Time (whichever is higher.) I don't have a link but I believe I saw it on AAO website. Try to use the Search filter.
