Wiki E/M w/injection mod 25?

I'm getting back into the E/M coding for RHC after two years. The office I will be coding for uses modifier 25 if any injection is given during an E/M visit. Is this correct? I don't remember doing this before.

Yes, modifier 25 on the E/M code when injection is also performed is correct, as long as the office visit documentation justifies the service is "above and beyond" what is required for the injection.
