Our podiatrist sees patients with ulcers in our office, checks the ulcer, and does a debridement if needed. Usually the debridement is through the dermal layer so I am coding 97597. I charge an office visit the first time he sees the patient, but if he does a debridement like 97597 at the follow up visit, I am generally not billing an office visit unless there is a new problem. The podiatrist does dictate everything needed for an E/M so should I also be coding an established patient office visit with the 97597?

Thanks for any help. I am new to wound care and struggling with it!
