Wiki E&M worksheets ???

I have been asking this question for years no answers yet. On a E&M worksheet for an ER visit.
There are 5 E&M codes,,,,, on the worksheets I have found there are only 4 columns listed,,,,

For the history and exam:
Problem focused
exp. problem focused
detailed
comprehensive

decision making:
minimal
low
moderate
high

Why is it not layed out in all 5 code set ups 99281 thr 99285 I always feel like a level is being skipped ?????
 
If you read the descriptions of each of the ED codes, you'll notice 99283 has expanded problem focused history and expanded problem focused exam, with moderate MDM. The history and exam are the same as 99282, but the MDM is the same as 99284, which tells me this is the "transitional" e/m code, if you will.

I wouldn't say a level is skipped, as all three key components must be met for any ED visit.
