E2325 Keeps Denying by Medicare because of Invalid Modifier

Has anyone ever gotten E2325 (Sip and Puff Accessory for a Wheelchair) paid by Medicare? It has been submitted with RR, but still denying for Missing, Invalid modifier. It's being billed as a Capped Rental. The LCD is not helpful either.
