How do I bill a Ebstein-Barr Virus IgG and IgM? The Ingenix CPT says:

86665: These tests may be ordered as EBV-EA titer, EBNA (IgG or IgM) titer, or EBV.VCA (IgG or IgM), or EB-VCA (IgG or IgM) titer.

So if this is for IgG or IgM does this mean that I only use this code once for both test, or do I report it twice? And if I report it twice to I append modifier 59 to the CPT? Thanks for the help!
