What are the documentation requirements for 33960 and 33961?
my provider documented the following, I was about to email him that ECMO daily visit is separately billable since he did not bill the 36822. However, I don't know what exactly he needs to document besides the this. Also he did document history, exam, labs (daily note)
"We are carrying out the care plan as agreed upon after the two family conferences yesterday. XXXX been on PC 35 (gives TV of ~ 400) PEEP 12 RR 26 and 100% for two hours and then we've incrementally decreased the ECMO O2 delivery. O2 saturations were 100% until ECMO O2 to 50% then O2 saturations to 85%, now the ECMO O2 is at room air With no sweep and flow of only 1 LPM. and her O2 saturations are 75%. ABG with worsening respiratory acidosis and PO2 of 50. XXX has mild respiratory effort but no other motion or signs of distress on current sedation regimen. XXXX answered. At this point I am not going to remove the catheter. Off vecuronium since yesterday.
