Hoping for some expert advice on good documentation to give the hardworking staff in the ED credit for their cognitive workload.

Assume the symptoms the patient presents with are not active and there are no findings. Say 5 labs were ordered and counseling was completed for return precautions. No diagnosis upon discharge. Total time in the ED 5 hours. The workload consists of a complete lab workup or rads and will never higher than a 2 because it only makes it in MDM column 2 as time cannot be used in the ED.
