That sounds right. I don't get notes either but have billed out both codes. So you really need to know what was done in order to bill correctly.

I had that too. I had to read the notes on the diagnostic report, and determined the patient wore it for 24 hours so it would be both awake and asleep.. so I coded for that. But i also came across one that there was no indication on the note and I had to query the doctor.
