43245 with 43999 OR 43270 with 43999

Which code is to be used for the below Procedure:

Patient following up after Roux-en-Y gastrojejunostomy

The gastrojejunal anastomosis was characterized by moderate stenosis and
dilation 4 cm (inner diameter) x 1 cm (length).
Coagulation for tissue destruction using argon plasma at 0.3
liters/minute and 70 watts was successful. The decision was made to use
sutures to decrease diameter of gastric outlet size. OverStitch device was attached to the proximal and distal
ends of the endoscope. The scope was reinserted. The device was loaded
with 2.0 polypropylene suture. A total of one sutures in purse string
fashion were placed with cinches on both ends over a 10mm balloon.

Interesting. Unlisted is correct for the overstich, but in this context. Almost looks like you could just code 43245 with 43270 and say 43999 is integral. Just a stitch is used to close things up at the end for the stenosis.

Or 43270 with 43999.
