EGD with replacement of Esophageal stent

Hello,

If provider is replacing esophageal stent due to anastomotic leak K91.89, is the removal of the stent included in placing a new stent? I was looking at 43247 for removal and 43266, 74360,26 for placing the new stent. Any help is appreciated.
