Wiki EKG and E/M (99213) with a 25 mod

Does anyone add the 25 modifier on an E/M visit and also has an EKG done? I have always added the 25 on the E/M code and was told yesterday not to as Blue Cross was denying visit with the 25 mod. Thanks.

I have recently started billing our EKG's with a mod -25 but was told I may need a -59 for an established patient

Thanks. We have gotten some denials with the 25 so I'll keep the 59 in mind for those situations.

This may be payer-specific, check with them. Some may want the 25 modifier, many don't if it's just an EKG in my experience.
