Patient is in for his Welcome to Medicare visit. If he has a diagnostic EKG, how does this affect the coding? Do you use the 93000 with diagnostic code which clashes with the G0402 or use the G0404/G0405 with Z13.6? Thanks.

If it is determined the EKG is outside of the "routine" screening (using the G0404/G0405;Z13.6) and the patient has a complaint or medical reason for the EKG, it is no longer a screening. You are going to assign whatever is driving the need for the test to the EKG. It depends on what the documentation says. Does the patient have a medical reason for the EKG to be run? As far as conflicting with the G0402, if your ICD10 codes are attached appropriately to the specific service, that should tell the story to the payer. But you would want to determine from the note if the patient presented with a reason for the EKG or is it just routinely performed as part of the WTM visit.
