When billing an EKG interpretation (93010) with either an initial inpatient or subsequent inpatient E&M service, an insurance carrier is denying the 93010 as inclusive. I am trying to find the guidelines that states these are included. Can anyone point me in the right direction. I have been spending all day looking at the CMS NCCI edits, Medicare claims processing manual and the NCCI edit manual. What am I missing?
