Wiki EKG's and echos- when to code and when not to

I am working in Practicode and for the life of me, I cannot figure out when to code an EKG and/or echo done during an ED visit and when not to. I've tried both ways and sometime it's right and sometimes it's not, and no explanation is given with the answer. Can anyone enlighten me? Is there a definitive answer? Thank you.
