Good morning,

I'm struggling with a proper code for the following procedure:

"After formal prep and drape of the right upper extremity, a posterolateral approach to the elbow was marked and executed with muscular retraction and arthrotomy yielding good exposure to the radial ulnar and ulnar humeral joint as well as the radiocapitellar joint. Fluid was cultured with swabs and periarticular synovium was also sent for cultures, Gram stain as well. He was noted that some gouty tophus was present in the joint capsule and that which was visible was excised. Joint was copiously irrigated with normal saline under gravity irrigation while moving the joint in both supination and pronation as well as flexion and extension. After this, the joint was treated topically within its anterior with vancomycin powder before closing with monofilament suture in layers."

The provider is wanting CPT code 24101 but I am torn between 24000, 24101 and 24102. This procedure was done for the possibility of elbow pyarthrosis but there was no bacteria growth on any gram stains or cultures afterwards so it was taken down to just the gouty trophus.

Can anyone help me out, I would greatly appreciate it.
 
24000 and 24101 are nearly identical and describe essentially the same thing.
The difference is .2 wRVU.
It is a meaningless and insignificant difference and there is zero need to belabor it.
Use 24101 because it has the marginally higher RVU value, but, seriously, there is no need to differentiate between these two codes.
