electronic signature - are all 3 elements required for accepted electronic signature - provider name, credential, date.

I have several notes that the credential is not included in the electronic signature but it is listed on the letterhead for the provider.

Credential and Signature

They do not have to co-exist in the same place in the signature,,, if the credential is on letterhead of the note or somewhere on the note, it is acceptable by CMS.
