If a patient is taking Coumadin as directed and their INR comes back extremely elevated what dx codes would be used to code this?

Is this for Coumadin management??
We used DX Z79.01 Long term (current) use of anticoagulants
of course you will need to confirm with payers for medical necessity. LCD's, NCD's etc..
